Journey: John's previous support provider struggled with quality engagement, resulting in limited progress and a lack of meaningful connection. When we began working together, we focused on establishing honest, purposeful goals that aligned with his personal aspirations. Through consistent, high-quality support and teamwork, we created a structured, step-by-step approach to achieving his goals. Along the way, we built confidence, belief in his abilities, and strengthened both his written and verbal communication skills, fostering greater independence and connection to his own life.
After 2.5 years of dedication and perseverance from John, he now successfully owns and operates his own business—an achievement reached despite significant challenges. Through determination, he has secured financial grants and engaged key stakeholders outside of the NDIS, leveraging additional support to turn his vision into reality.
Approach: A person-centered, participant-led support model, ensuring that goals are driven by the individual's needs and aspirations. Complex case management strategies are applied to carefully select and coordinate the right support workers, fostering consistency, skill development, and confidence. John also identifies as being an aboriginal man, and cultural competency and knowledge or working with aboriginal persons was a must at building a strong connection with John as well as linking in and ecouraging comunity connections. At the heart of this approach is a dignity-driven support system, empowering participant to grow and thrive.
